Transcript Release

A signed transcript release form is required of all students, in accordance with the Federal Education Right to Privacy Act (FERPA) before any academic records can be forwarded.  Please submit completed forms to the Counseling Office.

Course Change Request Form

If a student wishes to withdraw from a scheduled course or to change a level, a course change request form must be signed by the parent, teacher for any change, and additionally by the school principal if the change occurs after the 15th school day of the year.  Forms are available at and need to be returned to the Counseling Office.

Schedule Template

In order to support our students as they learn the new schedule, this template allows students to view their daily courses in an easy-to-read format.

High School Partnership (NCCC)

Program designed to provide qualified high school juniors and seniors the opportunity to enroll in up to two college courses each semester with no tuition fees.  Qualified students must have an overall academic average of 80% in order to be recommended by school personnel.  Students must meet pre-requisites for the course, and complete the Accuplacer test before enrolling.  Once selected for the program, students are notified directly by NCCC with enrollment information.

Course Planning Worksheet

This form allows students to visually plan out their four-year high school courses in order to meet both graduation requirements and to achieve their post-secondary goals.  During a student's individual planning meetings with the School Counselor, this form is completed and updated by the student as part of their Student Success Plan.
